Introduction : Case study: A 30-year-old woman from Uttarakhand underwent Single Incision Laparoscopic Cholecystectomy (SILS) by Dr. Mir Asif Rehman in Gurgaon. Learn how advanced scarless gallbladder surgery successfully treated her recurring right-side abdominal pain, gallstones, and fatty-food intolerance with fast recovery at our laparoscopic surgery centre in Delhi NCR.

Patient Profile & Medical History – recurrent Gallbladder Pain

Name: As per Patient (Confidential)

Age: 30 years

Patient Home Location: Uttarakhand

Chief Complaint:

Recurrent pain on the right side near the lower end of ribs for 2 months, especially after eating oily or fatty foods.

Patient Clinical History

The patient, a young 30-year-old woman, visited Dr. Mir Asif Rehman with a history of:

• Sharp pain in the right upper abdomen

• Pain radiating toward the back and shoulder

• Episodes occurring after meals, particularly after consuming fatty foods

• Associated nausea and bloating

• 2-3 attacks since 2 months

Due to increasing frequency of episodes and impact on daily life, she sought a permanent solution.

Diagnostic & clinical examination

After clinical examination, the following tests were advised by Dr Mir Asif:

1. Abdominal Ultrasound

• Presence of solitary gallstones

• Mild gallbladder wall thickening

• No CBD (Common Bile Duct) dilation

2. Blood Tests

• Liver function tests: Normal

• No signs of jaundice or infection

Diagnosis:

Symptomatic Gallstone Disease (Cholelithiasis) requiring surgical removal of the gallbladder.

Decision for Surgery

After the diagnostic evaluation, Dr. Mir Asif advised and counseled the patient about treatment options. Considering:

• Her young age

• Recurring, debilitating pain

• Desire for faster recovery

• Cosmetic concerns (minimal scarring)

Dr. Mir Asif opted for Single Incision Laparoscopic Cholecystectomy (SILS)—an advanced technique performed by only a few surgeons in India.

She traveled all the way from Uttarakhand to our Centre in Gurgaon specifically to undergo this minimally invasive, scarless surgery under Dr. Mir Asif Rehman.

Day prior to her surgery routine blood work was done so as to avoid surprise’s on the day of surgery and Pre Anesthesia consult was done to check for the feasibility of administration of general Anesthesia

The Day of Surgery

Single Incision Laparoscopic Cholecystectomy (SILS)

• Surgery performed through one tiny incision inside the belly button

• Specialized instruments used

• Complete removal of gallbladder with stones

• No drain tube required

• Blood loss was minimal

• Procedure time: ~25 minutes

• Patient was shifted to recovery in stable condition

Postoperative Recovery

• The patient was able to walk within a few hours

• Started taking liquids 6 hours post procedure and semisolid as soon as she was able to tolerate liquids

• Discharged within 24 hours

• Only one hidden scar inside the navel—virtually invisible

• Returned to routine activities in 3–4 days

• Follow-up ultrasound confirmed normal recovery

Patient Happiness and Surgery Outcome

The patient reported:

• Complete resolution of pain

• Ability to eat normally without discomfort

• High satisfaction with the scarless cosmetic result

• Grateful for a smooth and painless surgical experience

Gallstones: Causes, Prevention & Treatment Options in Gurgaon

By Dr. Mir Asif Rehman, Gallbladder & Laparoscopic Surgeon at Mirascare Clinics, Gurgaon

Millions of people worldwide suffer from gallbladder stones, also known as pith ki pathri in Hindi, one of the most prevalent digestive issues. We see a lot of patients with gallstone-related pain, indigestion, or discomfort at Mirascare Clinics in Gurgaon.

But what exactly are gallstones, why do they form, and how are they treated? Let’s understand.

What is the Gallbladder and Why Do Stones Form?

On the right side of your abdomen, directly beneath the liver, is a tiny organ called the gallbladder. Bile, a digestive fluid that aids in the breakdown of fatty foods, is stored there.

Gallstones can develop inside the gallbladder as a result of improper gallbladder function or excessively concentrated bile.

Common Causes of Gallstones

Gallstones are more common in certain groups of people. The major risk factors include:

Hormonal changes in women – Dr. Mir Asif says mostly women in their 30s and 40s are more likely to develop gallstones due to hormonal fluctuations.

Genetic factors – If your parents or siblings have gallstones, your risk is higher.

Infections – Some bacterial or viral infections can damage the gallbladder and lead to stone formation.

Blood disorders – Rarely, certain blood diseases can increase the risk of gallstones.

In fact, studies and medical practice show that if 10 gallbladder surgeries are done, around 7–8 patients are women and only 2–3 are men.

Symptoms of Gallstones

Doctor Says, Not all gallstones cause problems. Many patients may experience no pain at all from silent (asymptomatic) stones. But when symptoms do appear, they could include:

Pain in the upper right abdomen (especially after fatty meals)

Nausea or vomiting

Indigestion, bloating, or heaviness

Fever or jaundice in severe cases (due to complications)

Do All Gallstones Need Surgery?

There are two approaches to gallstone treatment:

Asymptomatic gallstones – If stones are not causing pain or problems, sometimes surgery is not immediately needed. Regular monitoring is enough.

Symptomatic gallstones – If stones cause pain, vomiting, or complications, surgery is the only effective treatment.

Because gallstones can cause serious complications if left untreated, some surgeons advise preventive surgery even for patients who are asymptomatic. Dr Mir asif says..

Modern Treatment Options at Mirascare Clinics, Gurgaon

Previously, a large incision was made during open surgery to remove the gallbladder (cholecystectomy). However, the gold standard for treating gallstones nowadays is laparoscopic surgery.

Types of Laparoscopic Surgery:

Conventional Laparoscopy – Four small incisions are made (at the belly button, above it, and two on the sides) to safely remove the gallbladder.

Single-Incision Laparoscopic Surgery (Speciality at Mirascare Clinics) – At Mirascare Clinics, Gurgaon, Dr. Mir Asif Rehman specializes in performing gallbladder surgery through a single 11 mm incision at the belly button.

What are the advantages of single-incision surgery?

Minimal pain and discomfort

Hardly visible scar (cosmetically superior)

Less risk of infection or bleeding

Quick recovery and faster return to normal life

How to Prevent Gallstones?

While not all gallstones can be prevented, you can reduce your risk by:

Maintaining a healthy weight (obesity increases risk)

Eating a balanced diet rich in fiber and low in unhealthy fats

Staying physically active

Avoiding crash diets or sudden weight loss

Why Choose Dr. Mir Asif Rehman at Mirascare Clinics, Gurgaon?

Over 7,500 successful laparoscopic and laser surgeries performed

Specialist in advanced gallbladder and gastrointestinal surgeries

Expertise in single-incision laparoscopic gallbladder surgery for faster recovery

Final Thoughts by Dr Mir Asif Rehman

Gallstones are a very common problem, especially among women in their 30s and 40s. While not all gallstones need immediate treatment, once they cause symptoms, surgery is the safest and most effective option.

Myth 1: Gallbladder surgery reduces life span

One of the most common misconceptions regarding gallbladder surgery is this one. In actuality, neither scientific research nor medical literature indicates that removing the gallbladder reduces life expectancy.

Every year, millions of people around the world safely have gallbladder surgery and return to normal, healthy lives.

Myth 2: Gallbladder surgery causes acidity or indigestion

The development of acidity or digestive issues following surgery is another frequent worry. In actuality, acidity is not a result of gallbladder removal.

Gallbladder surgery is not the only cause of acidity; there are numerous other factors as well, including lifestyle choices, stress, and diet.

In fact, you might have experienced pain, bloating, or discomfort when consuming fatty foods if you had a gallbladder that was symptomatic (with stones). This issue usually resolves after surgery, which facilitates digestion.

The Reality: Safe and Effective Treatment

The removal of the gallbladder is a simple, safe, and minimally invasive (laparoscopic) operation. When gallstones cause pain, infection, or other symptoms, it is advised.

You can live a better life without the constant pain and discomfort by having the diseased gallbladder removed.

Advanced SILS Method for Gallbladder Surgery

For gallbladder removal, Dr. Mir Asif Rehman employs the most cutting-edge SILS (Single Incision Laparoscopic Surgery) technique.

For the purpose of to insert the camera and surgical tools during traditional laparoscopic surgery, the surgeon typically makes three to four tiny abdominal incisions. Despite being safe and efficient, this procedure still leaves a few minor scars.

The SILS technique involves making a single, tiny incision, usually inside the belly button, to perform the entire procedure. This implies:

Only a single, hidden scar (often almost invisible)

Less pain after surgery compared to multiple incisions

Faster recovery and early return to normal activities

Better cosmetic outcome, since the scar is hidden in the natural fold of the navel

One of the most advanced methods of gallbladder surgery available today is SILS, a state-of-the-art, minimally invasive procedure that blends safety, efficacy, and aesthetic advantages.

What is Fundoplication? A Brief Overview

“Fundoplication surgery is a treatment employed to address gastro-oesophageal reflux disease (GERD), a disorder characterised by the recurrent regurgitation of stomach acid into the oesophagus, resulting in discomfort.” Dr Mir Asif, Renowned General and Laparoscopic Surgeon in Delhi NCR & Director of MIRASCARE Multi speciality Clinic, Gurugram

How Fundoplication Works?

The procedure entails encircling the upper section of the stomach (the fundus) around the lower oesophagus. This reinforcement establishes a valve mechanism that inhibits the reflux of stomach acid and contents into the oesophagus.

Types of Fundoplication

1. Nissen Fundoplication (360-degree wrap): The stomach is wrapped completely around the esophagus. It’s the most common and effective type.

According to Dr Mir, A renowned general and laparoscopic surgeon in India, says Gallstone disease, also known as cholelithiasis, is a common medical conditions in the pancreatobiliary system which solid particles or stones accumulate within the gallbladder or bile ducts. These stones can vary in size and content, causing a variety of symptoms and consequences.

Some Certain parts of the world have a higher prevalence of gallbladder stones than other areas. Even if you can consider it, gallbladder and biliary disease cases grew drastically from 26.35 million (95% UI: 22.79–30.60) in 1990 to 52.00 million (95% UI: 44.20–61.21) in 2019 (EAPC = 2.70 [95% CI: 2.58–2.81]). In India, it is estimated to be around 4%, while in the West, it is 10%.

The global burden of gallstone disease has been increasing over the years, and several factors contribute to this growing problem. Here are some key points to consider:

1. Prevalence:

Gallstone disease is one of the most common gastrointestinal illnesses in the world. It affects people of all ages and ethnicities, but it is more prevalent in women and persons over the age of 40.

2. Risk Factors:

On our website, we have already described how gallstones are caused by a variety of risk factors, including heredity, obesity, a high-fat diet, fast weight reduction, certain medical conditions (such as diabetes), and certain drugs. Obesity and metabolic syndrome are becoming more common in many nations, leading to an increased burden of gallstone disease.

Learn more about gall stone risk factor visit : https://www.mirascare.com/gallbladder-stone-treatment

3. Changing Diets:

High-fat, low-fiber diets have been linked to an increased risk of gallstone development. Gallstones are becoming increasingly common in various regions of the world as Western-style diets become more popular.

4. Obesity Epidemic:

The global obesity epidemic has a considerable impact on gallstone disease prevalence. Obesity can cause alterations in cholesterol metabolism and increased cholesterol release into bile, which can encourage gallstone development.

5. Healthcare Access:

Improved healthcare access and diagnostic capabilities have resulted in greater detection and reporting of gallstone disease patients in some locations. This, in turn, contributes to a larger recorded illness burden.

6. Complications:

Gallstones can induce acute cholecystitis, biliary colic, pancreatitis, and obstructive jaundice, among other consequences. These problems may necessitate hospitalization and surgical procedures, increasing the healthcare burden.

7. Aging Population:

The global population is aging, and gallstone disease is increasingly common in the elderly. Gallstone disease is projected to become more prevalent as the world’s population ages.

8. Emerging Economies:

Rapid economic development in emerging economies has resulted in lifestyle and dietary changes that may raise the risk of gallstone formation. Higher prevalence of gallstone disease have been linked to urbanization and a shift away from traditional diets and toward more Westernized ones.

9. Treatment Options :

Gallstone treatment may include sophisticated surgical removal of the gallbladder (cholecystectomy) techniques such as SILS Techniques or any other nonsurgical approaches such as medicines to dissolve stones. The treatment chosen is determined on the intensity of the symptoms as well as the patient’s overall health.

Dr. Mir says that after consultation with different types of people for gallstone treatment, it is critical to recognise that gallstone disease is a multifaceted disorder involving a complex interplay of hereditary, environmental, and lifestyle variables. Preventive measures like keeping a healthy weight, eating a well-balanced diet, and increasing physical exercise can help lower the risk of getting gallstones.

Medical and surgical treatments are available to patients suffering with symptomatic gallstones. The increasing global burden of gallstone disease emphasizes the critical relevance of public health measures targeted at preventing and controlling this widespread medical condition.

What is Perianal Abscess?

An illness called a perianal abscess collects pus in the tissues next to the anus. The anal glands, which are small glands adjacent to the anus that secrete mucus to lubricate the anal canal, are typically the cause of it.

The anal glands can get infected when they plug up with bacteria. This infection results in the formation of an abscess, which is a pus-filled pocket, while the body’s immune system fights the infection.

All ages, including infants, kids, and adults, are susceptible to developing perianal abscesses. They could be brought on by a number of things, such as:

Blocked Anal Glands: An environment favourable to bacterial development and infection is produced when the anal glands become closed.

Anal Fissures: These are minute tears or fissures in the anal canal’s lining. They may aid in the formation of an abscess if they contract an infection.

Crohn’s disease: Due to the persistent inflammation and immune system malfunction brought on by this inflammatory bowel disease, people with Crohn’s disease are more likely to develop perianal abscesses.

Typical signs of a perianal abscess include:

• Redness and swelling around the anus.

• Tenderness and pain in the affected area.

• In certain situations, fever and overall malaise.

• Trouble sitting or discomfort when going to the toilet.

A fistula, a tiny tube connecting the abscess to the skin close to the anus, can develop into a perianal abscess if it is not treated. Constant discharge and recurring infections are two effects of fistulas.

What is Emergency Case of Perianal Abscess?

When a perianal abscess develops severe and needs rapid medical attention, it is referred to be an emergency case. A small collection of pus that develops close to the anus or rectum is known as a perianal abscess. It frequently results from an infection that is brought on by the anal glands becoming blocked or obstructed.

A perianal abscess may occasionally develop into an emergency for the following reasons:

Extreme Pain: An abscess may produce excruciating pain that makes it impossible for the individual to sit, walk, or carry out daily tasks.

Rapidly Worsening Symptoms: If the abscess’s swelling, redness, and soreness suddenly get worse, it could be a sign that the infection is spreading or that complications are developing.

High fever, chills, and general malaise: These symptoms point to a systemic infection that may need immediate medical attention.

Stool or Urine Passing Difficulties: If the abscess grows and exerts pressure on the adjacent structures, it may become difficult to pass stools or urine. This can indicate a deeper, more serious problem.

Spreading Infection: In some situations, an abscess-related infection can spread to nearby tissues or the bloodstream, resulting in sepsis. Sepsis is a potentially fatal illness that needs quick medical attention.

If you suspect you have a perianal abscess that has to be treated right away or show any of the symptoms listed above, you must seek medical attention right once. A medical professional can assess the situation, provide the necessary care, and help to prevent complications because of the abscess.

Treatment for perianal abscess

A mix of medicinal and surgical procedures are frequently used to treat a perianal abscess. The major objectives of treatment are to promote healing, drain the abscess, and get rid of the infection. Here are some typical methods:

Drainage and Incision

Antibiotics and pain relief

Post-Procedure Care

A tiny incision is typically used to drain the pus from a perianal abscess. A general and laparoscopic surgeon usually does this. If there is evidence of a severe infection or if a fistula is present, antibiotics may occasionally be recommended.

If you think you may have a perianal abscess, you should contact a doctor right away since timely care can ease your symptoms, stop more problems, and speed up the healing process.

As per the needs of patients, medical technology has gone through many improvisations. One such technology improvisation is laparoscopic surgery.

With the help of the latest equipment, doctors can have a detailed inspection of your internal organs. This is why there is a huge demand for a top laparoscopic surgeon in Gurgaon.

Why laparoscopic surgery is called so?

Do you know why does laparoscopy has this name? The term laparoscopic is composed of two Greek words- lapara and scopio. Lapara and scopio mean side or flank and watch respectively.

This means that this kind of surgery is used to see the inner side of the body. The special equipment used for watching the internal affected organ of the patient’s body is known as a laparoscope. A high-quality laparoscope is the primary attribute of any best laparoscopy hospital in Gurgaon.

How is laparoscopy conducted?

As mentioned earlier, laparoscopy provides the surgeon with an internal view of the patient’s affected organ. The whole procedure of laparoscopic surgery can be illustrated in simple steps as follows.

Incision near the affected area

First of all, a small incision is made in the patient’s body to make way for inserting the laparoscope.

Insert the laparoscope

Either a long rod or a digital laparoscope is put inside via these incisions. Nowadays, laparoscopes are equipped with good qualityoptical cables that provide clear imaging of organs.

Injection of suitable gas

After the insertion laparoscope into the body, a small amount of gas is released in the affected part of the body to inflate the organs. Doctors use carbon dioxide gas as it is non-inflammable. It is also absorbed by blood vessels that can be further exhaled easily. This gas provides a clear space for better visibility of organs.

Observation of affected organ by laparoscope

The laparoscope has a digital camera at the end. This conveys the internal picture of the organ to the outer world. The surgeon then monitors the affected area and looks for the deceased part of the organ.

Making of additional incisions

When the surgeon watches and confirms the state of the affected organ, preparations for the next step begin. Based on the location of the damaged organ, additional incisions are made via which other arrangements are put inside the body.

Remove the damaged part

Special equipment is inserted via the additional incisions. With continuousmonitoring of the inner organs fromthe laparoscope, the surgeon removes the defective organ or part.

Sealing of incisions

After the damaged part gets removed, the carbon dioxide gas is exhaled via a small knob of the laparoscope. All the injectedequipment is then taken out of the body. With the help of skin glue and stitches, incisions are joined back.

How many types are there for a laparoscopy?

Laparoscopic surgery can treat a defined set of medical complications. Due to this variety, it is not difficult to find a laparoscopic surgeon in Gurgaon. However, most of the types of laparoscopy surgeries deal with digestive system complications. It sometimes finds many applications in gynecological diagnosis also. Here is a list of types of laparoscopic surgeries.

Stomach treatment

There are cases when an unwanted tumor gets developed in the stomach. This excessive cluster of cells damages the normal functioning of the stomach. Laparoscopic surgery is the easiest way to remove this kind of stomach tumor from the body.

Appendectomy

The appendix is the end part of our large intestine that stores good bacteria. Due to some unwanted reasons, the entry of this appendix gets blocked or interrupted which leads to invasion. A swollen up appendix is believed to be very harmful as it can rupture anytime.

A rupture of such an appendix has fatal consequences thus it needs to be treated as soon as possible. In such a case, doctors use a laparoscope to have a look at the internal conditions and then remove the affected part by laparoscopic surgery.

Colon Treatment

There are cases where certain outgrowths get developed in the colon which cannot be removed by regular colonoscopy. These outgrowths are called polyps and are best treated by laparoscopy.

GERD sphincter fixing surgery

GERD stands for Gastroesophageal reflux disease. People suffering from GERD have a weak sphincter. This results in unwanted backflow of acid that creates a nausea-like feeling. To cure this, doctors prefer laparoscopic surgery. With the help of a laparoscope and other essential equipment, the wakenedsphincteris fixed to minimize the effect of GERD.

Gall bladder removal / GallStone Treatment

The gall bladder is an essential part of our human digestive system. It stores bile that is secreted by the liver for digesting the food. Sometimes, this bladder gets damaged due to the accumulation of crystals.

To cure this condition, doctors prefer removing the affected gall bladder. The easiest and comfortable way of doing so is laparoscopic surgery.

Hernia Surgery

The hernia is a condition in which outgrowths of tissues block the passage between the abdomen and reproductive system. This leads to an interruption in blood flow through the affected part of the body. This condition causes severe pain and discomfort to the customer.

It is best cured by laparoscopic surgery. With the help of small incisions, the damaged part is monitored and analyzed proficiently. The unwanted outgrowth is then removed with the use of special tools.

How is laparoscopic surgery better from other methods of treatment?

In most of the applications of laparoscopic surgery, other alternatives such as open surgery are also possible. This is due to the following advantages of laparoscopic surgery.

Unlike all other surgery methods, laparoscopy surgery involves small incisions that get recovered easily.

Laparoscopic treatment is found to have a high chance of success than any other surgery method.

This method involves fewer cuts and thus requires less medication which effectively improves the patient’s experience.

Open laser surgery has a high risk of infections. Laparoscopic surgery is preferred over open surgery because of less exposure of internal organs to the outer environment.
