Learn how advanced scarless gallbladder surgery successfully treated her recurring right-side abdominal pain, gallstones, and fatty-food intolerance with fast recovery at our laparoscopic surgery centre in Delhi NCR.

Patient Profile & Medical History – recurrent Gallbladder Pain

Chief Complaint:

Recurrent pain on the right side near the lower end of ribs for 2 months, especially after eating oily or fatty foods.

Patient Clinical History

The patient, a young 30-year-old woman, visited Dr. Mir Asif Rehman with a history of:

• Sharp pain in the right upper abdomen

• Pain radiating toward the back and shoulder

• Episodes occurring after meals, particularly after consuming fatty foods

• Associated nausea and bloating

• 2-3 attacks since 2 months

Due to increasing frequency of episodes and impact on daily life, she sought a permanent solution.

Diagnostic & clinical examination

After clinical examination, the following tests were advised by Dr Mir Asif:

1. Abdominal Ultrasound

• No signs of jaundice or infection

Diagnosis:

Symptomatic Gallstone Disease (Cholelithiasis) requiring surgical removal of the gallbladder.

Decision for Surgery

After the diagnostic evaluation, Dr. Mir Asif advised and counseled the patient about treatment options. Considering:

• Her young age

• Recurring, debilitating pain

• Desire for faster recovery

• Cosmetic concerns (minimal scarring)

Dr. Mir Asif opted for Single Incision Laparoscopic Cholecystectomy (SILS)—an advanced technique performed by only a few surgeons in India.

She traveled all the way from Uttarakhand to our Centre in Gurgaon specifically to undergo this minimally invasive, scarless surgery under Dr. Mir Asif Rehman.

Day prior to her surgery routine blood work was done so as to avoid surprise’s on the day of surgery and Pre Anesthesia consult was done to check for the feasibility of administration of general Anesthesia

The Day of Surgery

Single Incision Laparoscopic Cholecystectomy (SILS)

• Surgery performed through one tiny incision inside the belly button

• Specialized instruments used

• Complete removal of gallbladder with stones

• No drain tube required

• Blood loss was minimal

• Procedure time: ~25 minutes

• Patient was shifted to recovery in stable condition

Cholecystectomy, another name for gallbladder surgery, is one of the most common surgeries done worldwide. However, a lot of patients and their relatives frequently worry:

“Will my life span become shorter after gallbladder removal?”

“Will I start having acidity or digestive issues after the surgery?”

Here Dr Mir Asif Rehman clear these doubts with facts.

Myth 1: Gallbladder surgery reduces life span

One of the most common misconceptions regarding gallbladder surgery is this one. In actuality, neither scientific research nor medical literature indicates that removing the gallbladder reduces life expectancy.

Every year, millions of people around the world safely have gallbladder surgery and return to normal, healthy lives.

Myth 2: Gallbladder surgery causes acidity or indigestion

The development of acidity or digestive issues following surgery is another frequent worry. In actuality, acidity is not a result of gallbladder removal.

Gallbladder surgery is not the only cause of acidity; there are numerous other factors as well, including lifestyle choices, stress, and diet.

In fact, you might have experienced pain, bloating, or discomfort when consuming fatty foods if you had a gallbladder that was symptomatic (with stones). This issue usually resolves after surgery, which facilitates digestion.

The Reality: Safe and Effective Treatment

The removal of the gallbladder is a simple, safe, and minimally invasive (laparoscopic) operation. When gallstones cause pain, infection, or other symptoms, it is advised.

You can live a better life without the constant pain and discomfort by having the diseased gallbladder removed.

Advanced SILS Method for Gallbladder Surgery

For gallbladder removal, Dr. Mir Asif Rehman employs the most cutting-edge SILS (Single Incision Laparoscopic Surgery) technique.

For the purpose of to insert the camera and surgical tools during traditional laparoscopic surgery, the surgeon typically makes three to four tiny abdominal incisions. Despite being safe and efficient, this procedure still leaves a few minor scars.

The SILS technique involves making a single, tiny incision, usually inside the belly button, to perform the entire procedure. This implies:

Only a single, hidden scar (often almost invisible)

Less pain after surgery compared to multiple incisions

Faster recovery and early return to normal activities

Better cosmetic outcome, since the scar is hidden in the natural fold of the navel

One of the most advanced methods of gallbladder surgery available today is SILS, a state-of-the-art, minimally invasive procedure that blends safety, efficacy, and aesthetic advantages.

What is Fundoplication? A Brief Overview

“Fundoplication surgery is a treatment employed to address gastro-oesophageal reflux disease (GERD), a disorder characterised by the recurrent regurgitation of stomach acid into the oesophagus, resulting in discomfort.” Dr Mir Asif, Renowned General and Laparoscopic Surgeon in Delhi NCR & Director of MIRASCARE Multi speciality Clinic, Gurugram

This operation is also conducted to rectify hiatal hernias, in which a segment of the stomach protrudes through the diaphragm into the thoracic cavity.

How Fundoplication Works?

The procedure entails encircling the upper section of the stomach (the fundus) around the lower oesophagus. This reinforcement establishes a valve mechanism that inhibits the reflux of stomach acid and contents into the oesophagus.

Types of Fundoplication

1. Nissen Fundoplication (360-degree wrap): The stomach is wrapped completely around the esophagus. It’s the most common and effective type.

2. Partial Fundoplication (270-degree or less wrap):

Toupet Fundoplication: A 270-degree posterior wrap.

Dor Fundoplication: A 180–200-degree anterior wrap.

These partial wraps are used when a full wrap might lead to complications like difficulty swallowing.

Indications for Fundoplication Surgery

Chronic GERD not managed by medications or lifestyle changes.

Complications of GERD, such as esophagitis or Barrett’s esophagus.

Due to Large hiatal hernias patient cause severe symptoms.

Persistent regurgitation or aspiration.

Our Surgical Process at MIRASCARE Centre, Gurgaon

Laparoscopic Fundoplication (Minimally Invasive): Dr. Mir Asif’s expertise in this minimally invasive surgery, which involves small incisions and quicker recovery times, is impressive.

Open Fundoplication: This is very traditional method compared to above Laparoscopic Fundoplication , Performed through a larger abdominal incision, typically reserved for complex cases.

What are Benefits of this Surgery

Significant reduction or elimination of GERD symptoms.

Prevention of esophageal damage.

Improved quality of life.

Risks and Side Effects

Like many other surgeries, this surgery has some side effects, but not very long-term (depending upon the person-to-person recovery process).

Difficulty swallowing (temporary or long-term).

Gas-bloat syndrome (inability to burp or vomit).

Recurrence of GERD symptoms in some cases.

Surgical complications like bleeding or infection.

What is the Recovery time?

Most patients recover within 2–4 weeks after the surgery performed successfully.

Dietary modifications, such as starting with liquids and gradually introducing solid foods, are essential during the healing process.

YES – EXTREMELY! Strangulated hernias can prove fatal and is a very serious condition.

It is a surgical emergency. It requires urgent surgical intervention and patients tend to do less well after emergency surgery than they do following planned hernia procedures.

“Mortality risk following elective hernia repair is low, even at an advanced age.
An emergency operation for strangulated hernia carries a substantial mortality risk.”

According to Swedish Research,

In case of an Emergency operation the Risk of Mortality is 7 times higher, while if bowel resection was undertaken, the risk of mortality increased by 20 times.

Evidence shows it is prudent to prevent strangulation and avoid emergency surgery at all cost.

It is absolutely necessary to get hernia treated as soon as possible before this dreaded complication sets in.

What is a strangulated hernia?

Abdominal wall hernias occur because of weakness in the abdominal wall, allowing intestine and other organs (small bowel, large bowel, omentum etc.) to come out through the opening.

Which in turn stops the blood vessels inside the intestine which result in the death of tissues quite rapidly and release of toxins in the blood stream leading to quick deterioration of overall health and can also lead to death.

What warning symptoms will I get?

The warning comes very suddenly, from nothing at all. The first thing to happen is usually acute pain.

That pain rapidly become severe and nothing relieves it. It is common for this onset to be measured in minutes and emergency surgical intervention is required.

Most commonly strangulation occurs in smaller hernias as the large hernias tend to slide in and out easily, and so are at less risk of being shut by the tough structures surrounding the hernia

What should alert you?

Hernia when associated with

Sharp or Severe pain

Vomiting

Blood in Excrement

Constipation

Malaise with or without a fever

A burning or hot sensation around the hernia

How is Strangulated Hernia treated?

A strangulated hernia repair is a Surgical emergency.

The surgeon’s priority is to save the life, save the organ entrapped and repair the hernia in that order.

‘The entrapped organ is put back where it belongs in the abdominal cavity after it is freed from the tough surrounding structures constituting hernia, which must be done before the tissue is permanently damaged.

Once the entrapped tissues are put back safely where they belong, the proper approach would be to repair the hernia immediately so as to prevent it happening again. Sadly, being an emergency, it is commonly the case that the surgeon would stop just at reliving the obstruction as a prolonged surgical repair of hernia would further endanger the life of the patient. Having ‘reduced’ the hernia the patient is discharged from the hospital in the hope that hernia is repaired at a later stage in a more controlled setting and before it strangulates again.

This is as bad a situation as it sounds.

How can you avoid strangulation of hernia?

Only by getting hernia treated before it gets that bad.

Strangulation can happen at any time. You do not want strangulation to happen when away on holiday or at any time away from high-quality surgical help.

What should you do if you suspect hernia has strangulated?

If you are experiencing sharp pain, pain which is not getting relieved by oral pain medication, you need to visit or consult a hernia specialist as soon as possible. You need to avoid strangulating of hernia at all costs, that can only be achieved by treating hernia as soon as you are diagnosed with this condition.

Getting a surgery done is a big decision for any individual who is suffering from this condition. It usually is tempting to put it off. Before you decide not to get the surgery done, you must understand the risks and benefits of your decision.

Reasons behind why you should not delay Hernia Repair Surgery

Hernias will not go away by themselves there is no medical, herbal or alternative remedy available for hernia. Surgery is the only way to repair your hernia condition. A hernia repair returns the organ or structure to its proper place and fixes the weakened area of muscle or tissue with the help of a mesh.

Getting a surgery done is a big decision for any individual who is suffering from this condition. It usually is tempting to put it off. Before you decide not to get the surgery done, you must understand the risks and benefits of your decision. Here are some important things to consider and discuss with your hernia specialist while making the decision.

How is the Hernia affecting your life?

The first thing to observe is whether or not you are experiencing any symptoms. Not everyone feels symptoms with a hernia, especially if you have small defect/defects. The most common symptom to occur with hernia is pain. Other symptoms can be feeling of heaviness, lump and fullness in the belly or groin area. Whether you are facing any symptoms or not, a hernia can affects your daily activities and the office work.

Delaying surgery may cause long time away from your work and fun, which you could be doing otherwise.

If you have any above-mentioned symptoms, especially pain, your doctor is likely to recommend surgery.

What if you don’t have any symptoms or they are minimal?

If you don’t have symptoms or symptoms are minimal even then surgery is recommended as complication arising from hernia can be dangerous and sometimes life threatening.

What are the Risks of Waiting and not getting the surgery done sooner?

Hernias can become incarcerated:

One of the possible serious risk of not fixing a hernia is that it can become trapped outside the abdominal wall—or get incarcerated.

Strangulation:

After hernia gets trapped outside the abdomen wall this can cut off the blood supply to the hernia and obstruct the bowel, resulting in a strangulation. This requires urgent surgical repair.

Avoiding an emergency situation is the main reason not to delay hernia surgery.

Hernias grow.

The most common scenario is that your hernia will continue to grow and weaken the surrounding tissues as the time passesby and this condition is very likely to increase your hernia symptoms, including pain, and may cause lifestyle changes. Smaller hernias are easier to repair than larger hernias.

We at Mirascare, along with our doctor’s would recommend upfront surgery instead of delaying, which can potentially prevent your symptoms from getting worse and avoids serious life threating complication. It can also help you avoid losing work or missing daily activities.

Hernias require surgery in time:

Even if you aren’t having symptoms, you may still want to consider having surgery sooner rather than later.

Hernia Surgery is rather unavoidable in most of the cases. Research shows that most people with hernia conditions have surgery within 10 years. Always Keep in mind that delaying surgery until your hernia is larger and the muscles are weaker may make surgery and recovery more difficult and reoccurrence more likely.

Your overall health may change.

Your age is the most important factor while determine whether waiting is a risk for you. Your overall health or physical shape may change over years and usually not for good. This will also affect your surgery and post recovery time.

Having surgery at a younger age and when the hernia is small will yield a better outcome and with lesser complications. Or even if you are elderly hernia repair is still beneficial as it averts life threating complications and will be recommended by the surgeons.

Making the right Decision for Hernia treatment

In today’s busy life any Surgery is inconvenient. Making the right choice or decision at the right time and not to delay your hernia repair will be beneficial for you.

You can schedule your surgery at your convenience with the help of our hernia specialist’s. We will guide you whether you are a right candidate for laparoscopic hernia surgery or not, After Laparoscopic Hernia surgery you’ll be back to routine activity and work sooner compare to Open Surgery. Surgery should be easier on you and recovery will likely be smoother at a younger age and with a smaller hernia.

Laparoscopic surgery: In Laparoscopic surgery very small cuts or incision(5 to 10mm) are made, you need to stay only 1 to 2 days in the hospital, and recovery time is very fast compared to open surgery. You can be back to work or normal routine activity after 2 to 3 weeks.

हर्निया होने के बहुत सारे कारण है।

इंसिज़नल हर्निया or Incisional hernia

1. ओपन हर्निया सर्जरी or Open Hernia Surgery

2. लेप्रोस्कोपिक सर्जरी or Laparoscopic Surgery/Keyhole Surgery

ओपन हर्निया सर्जरी or Open Hernia Surgery

(Minimal Scar)

As per the needs of patients, medical technology has gone through many improvisations. One such technology improvisation is laparoscopic surgery.

With the help of the latest equipment, doctors can have a detailed inspection of your internal organs. This is why there is a huge demand for a top laparoscopic surgeon in Gurgaon.

Why laparoscopic surgery is called so?

Do you know why does laparoscopy has this name? The term laparoscopic is composed of two Greek words- lapara and scopio. Lapara and scopio mean side or flank and watch respectively.

This means that this kind of surgery is used to see the inner side of the body. The special equipment used for watching the internal affected organ of the patient’s body is known as a laparoscope. A high-quality laparoscope is the primary attribute of any best laparoscopy hospital in Gurgaon.

How is laparoscopy conducted?

As mentioned earlier, laparoscopy provides the surgeon with an internal view of the patient’s affected organ. The whole procedure of laparoscopic surgery can be illustrated in simple steps as follows.

Incision near the affected area

First of all, a small incision is made in the patient’s body to make way for inserting the laparoscope.

Insert the laparoscope

Either a long rod or a digital laparoscope is put inside via these incisions. Nowadays, laparoscopes are equipped with good qualityoptical cables that provide clear imaging of organs.

Injection of suitable gas

After the insertion laparoscope into the body, a small amount of gas is released in the affected part of the body to inflate the organs. Doctors use carbon dioxide gas as it is non-inflammable. It is also absorbed by blood vessels that can be further exhaled easily. This gas provides a clear space for better visibility of organs.

Observation of affected organ by laparoscope

The laparoscope has a digital camera at the end. This conveys the internal picture of the organ to the outer world. The surgeon then monitors the affected area and looks for the deceased part of the organ.

Making of additional incisions

When the surgeon watches and confirms the state of the affected organ, preparations for the next step begin. Based on the location of the damaged organ, additional incisions are made via which other arrangements are put inside the body.

Remove the damaged part

Special equipment is inserted via the additional incisions. With continuousmonitoring of the inner organs fromthe laparoscope, the surgeon removes the defective organ or part.

Sealing of incisions

After the damaged part gets removed, the carbon dioxide gas is exhaled via a small knob of the laparoscope. All the injectedequipment is then taken out of the body. With the help of skin glue and stitches, incisions are joined back.

How many types are there for a laparoscopy?

Laparoscopic surgery can treat a defined set of medical complications. Due to this variety, it is not difficult to find a laparoscopic surgeon in Gurgaon. However, most of the types of laparoscopy surgeries deal with digestive system complications. It sometimes finds many applications in gynecological diagnosis also. Here is a list of types of laparoscopic surgeries.

Stomach treatment

There are cases when an unwanted tumor gets developed in the stomach. This excessive cluster of cells damages the normal functioning of the stomach. Laparoscopic surgery is the easiest way to remove this kind of stomach tumor from the body.

Appendectomy

The appendix is the end part of our large intestine that stores good bacteria. Due to some unwanted reasons, the entry of this appendix gets blocked or interrupted which leads to invasion. A swollen up appendix is believed to be very harmful as it can rupture anytime.

A rupture of such an appendix has fatal consequences thus it needs to be treated as soon as possible. In such a case, doctors use a laparoscope to have a look at the internal conditions and then remove the affected part by laparoscopic surgery.

Colon Treatment

There are cases where certain outgrowths get developed in the colon which cannot be removed by regular colonoscopy. These outgrowths are called polyps and are best treated by laparoscopy.

GERD sphincter fixing surgery

GERD stands for Gastroesophageal reflux disease. People suffering from GERD have a weak sphincter. This results in unwanted backflow of acid that creates a nausea-like feeling. To cure this, doctors prefer laparoscopic surgery. With the help of a laparoscope and other essential equipment, the wakenedsphincteris fixed to minimize the effect of GERD.

Gall bladder removal / GallStone Treatment

The gall bladder is an essential part of our human digestive system. It stores bile that is secreted by the liver for digesting the food. Sometimes, this bladder gets damaged due to the accumulation of crystals.

To cure this condition, doctors prefer removing the affected gall bladder. The easiest and comfortable way of doing so is laparoscopic surgery.

Hernia Surgery

The hernia is a condition in which outgrowths of tissues block the passage between the abdomen and reproductive system. This leads to an interruption in blood flow through the affected part of the body. This condition causes severe pain and discomfort to the customer.

It is best cured by laparoscopic surgery. With the help of small incisions, the damaged part is monitored and analyzed proficiently. The unwanted outgrowth is then removed with the use of special tools.

How is laparoscopic surgery better from other methods of treatment?

In most of the applications of laparoscopic surgery, other alternatives such as open surgery are also possible. This is due to the following advantages of laparoscopic surgery.

Unlike all other surgery methods, laparoscopy surgery involves small incisions that get recovered easily.

Laparoscopic treatment is found to have a high chance of success than any other surgery method.

This method involves fewer cuts and thus requires less medication which effectively improves the patient’s experience.

Open laser surgery has a high risk of infections. Laparoscopic surgery is preferred over open surgery because of less exposure of internal organs to the outer environment.

How Hiatal Hernia and Gastroesophageal Reflux Disease are related?

If someone has Hiatal Hernia chances are he or she may be also be suffering from Gastroesophageal reflux disease or the other way around. It is common, several types of research show Hiatal Hernia is associated with GERD or acid refluxes.

A Hernia is a small bulge that occurs when an internal organ pushes through a weakness in the muscle or surrounding tissue wall.

A Hernia occurs usually between the chest and hips. And Hiatal Hernia occurs when the upper part of the stomach moves up into the chest and pushes through the diaphragm (diaphragmatic hiatus). The diaphragm is the muscle separating the abdomen from the chest.

Gastroesophageal Reflux Disease (GERD) or acid reflux occurs when gastric acid frequently flows back into the tube called the esophagus which connects the mouth and stomach.

What is the Reason for Gastroesophageal Reflux

The reason for Gastroesophageal Reflux happens when the lower esophageal sphincter (LES) is weak or relaxes wrongly. The LES opens during normal digestion to allow food to pass into the stomach and closes to prevent the flow back of food and acidic stomach juices into the esophagus.

A Hiatal Hernia can weaken the LES and increase the risk for Gastroesophageal reflux.

The opening in the diaphragm muscle supports the lower end of the esophagus. Many patients with a Hiatal Hernia will not have problems with heartburn or acid reflux. But having a Hiatal Hernia may allow stomach contents to reflux more easily into the esophagus causing discomfort.

Read More about Hernia Treatment & Surgery in Gurgaon

Hiatal Hernias are often asymptomatic. One may not be aware of their condition until the protrusion of the stomach through the hiatus is quite large.

What are the Symptoms?

However, larger Hiatal Hernias are big enough to allow undigested food and stomach acids to reflux into the esophagus. This means that people with Hiatal Hernia are likely to experience the symptoms of GERD. These include:

Heartburn

Dysphagia (trouble swallowing)

Frequent burping

Sore throat

One must note acid reflux can be caused because of a wide variety of underlying factors other than the Hiatal Hernia. If you have acid refluxes more than twice a week, you must see a doctor. A doctor can diagnose Gastroesophageal Reflux Disease based on a physical examination or other tests.

What is Surgical and Non-Surgical Treatment?

Home Treatment

If someone has this type of Hernia they can have more uncomfortable acid refluxes at night as when someone lies down the acid moves easily into the esophagus.

One solution is to modify the diet.

Avoiding fried, fatty foods,

and limiting caffeine intake can help but this may not be a long-term solution.

Some anti-acid tablets can relieve symptoms of GERD, However, long term use could result in side effects.

For the vast majority of people with GERD and Hiatal Hernia conditions, symptoms can often be effectively managed with a combination of lifestyle changes and medication. But few patients may require laparoscopic surgery for Hiatal Hernia.

Surgical Treatment

A laparoscopic surgeon can treat Hiatal Hernia by a minimally invasive surgery called fundoplication. This will keep the stomach from bulging through the diaphragm opening and to prevent stomach acid from flowing into the esophagus.

What are the Benefit of laparoscopic Surgery?

Get relief from symptoms

Don’t have to continue anti-acid medicines anymore
